Welcome to Life Chiropractic and Family Health & Wellness Center of Pembroke Pines, Inc. We are honored that you have entrusted your care to us and assure you that we will work hard to meet and exceed your expectations.

For 25 years we have treated patients from all over South Florida, the Caribbean and Latin America. Regardless of whether patients are referred by their family, friends, co-workers, physicians or insurance companies, they will be impressed by the stat-of-the-art diagnostic, rehab and chiropractic equipment and the outstanding knowledge and skills of Dr. Hodish and his staff.

At the Family Health and Wellness Center you and your family will be given care like you are our own family.

Or, if you were injured in some form of accident that will be filed with Automobile (PIP) Insurance, Workers Compensation, or an Attorney then click, print and fill out Patient Basic Information Form:

Then bring this completed form with you, and:

• Photo ID
• Insurance Cards
• Pertinent Medical Records
• X-ray / MRI Films and Reports
